If you sponsor, you may request a receipt for tax deduction and recognition on a sponsor banner and the @Downtownoxnard social media. Please submit this form (with payment applicable) in person or email by Monday, September 29th to: Downtown Oxnard, 519 South C Street, Oxnard, CA 93030 Email: info@downtownoxnard.org HALLOWEEN EVENT PARTICIPANT RELEASE I hereby agree to participate in the DOIA Halloween Haunt in the Park to be held on October 25, 2025. I hereby allow the DOIA to use my own name, image, likeness and registration information for the purposes of publicity – both before and after the actual event. I hereby waive, release, and hold Harmless Downtown Oxnard Improvement Association (“DOIA”), City of Oxnard, their directors, employees, sponsors, officials, volunteers, and event participants (“Release Parties”) from all claims by me for any liability, injury, loss or damage in any way connected with my participation in the event. I HAVE READ THIS WAIVER AND RELEASE CAREFULLY AND HAVING DONE SO I AM SIGNING IT VOLUNTARILY ON BEHALF OF MYSELF.
